This form should only be submitted once by each club, please ensure you have all of the relevant information ready before entering the data. This form must be submitted by 5th October 2012. This will help all captains in the organisation of the mini leagues. As per EQU 1.8, one league point per week will be deducted from all teams failing to adhere to this regulation.

All captains' details will be circulated via e-mail on Tuesday 9th October.
